Clascoterone: The First New Hair Loss Mechanism in Nearly 30 Years
For nearly 30 years, the pharmacological toolkit for androgenetic alopecia has worked through two mechanisms: blocking DHT production (finasteride, dutasteride) or promoting hair growth independently (minoxidil). A third mechanism is now entering the final stages of clinical development — and it works at a fundamentally different point in the process.
01What clascoterone is
Clascoterone is a topical androgen receptor antagonist — it blocks DHT from binding to the androgen receptor at the scalp, rather than reducing DHT production systemically. It's already FDA-approved as Winlevi cream for acne; the hair-loss formulation (Breezula) is a higher-concentration scalp solution in late-stage trials with regulatory submission expected around 2026–2027.
Finasteride reduces DHT production. Clascoterone blocks DHT from reaching its receptor — a downstream intervention. Even if DHT is present, it can't miniaturize the follicle if the receptor is blocked. This means clascoterone can work alongside finasteride (different points of intervention) and may work for people who can't take systemic DHT-reducing drugs.
02The Phase 2 evidence
Phase 2 trials showed statistically significant improvement in hair count vs. placebo. The topical delivery means systemic anti-androgen effects are minimal — no estrogen imbalance, no reproductive concerns that come with systemic anti-androgens. This makes it particularly interesting for women (who often can't use finasteride) and for men who've experienced finasteride side effects.
03What to do now
Clascoterone is not yet available for hair loss (pending regulatory approval). The evidence-backed options available today remain the right starting point — and the hair you protect now with current treatments is the foundation a new approval would build on.
